The aim is to commence BiPAP at settings of 12 cmH2O/4cmH2O (IPAP of 12, EPAP of 4) and escalate the IPAP. The substantive therapy often includes reopening coronary arterial circulation and inotropic support. Usual settings are 5, 7.5 or 10 cmH20 BiPAP. PEEP or CPAP also allows oxygen to reach more alveoli. By addressing these issues using carefully titrated end-expiratory pressure, a significant amount of overall stabilization takes place. PEEP or CPAP helps to reopen alveoli, making it easier for the patient to breathe. Some patients experience respiratory acidosis due to the inability to maintain adequate minute volume due to the shift of the compliance curve. Multiple studies have shown non-invasive ventilation (NIV), specifically, continuous positive airway pressure (CPAP) to be efficacious in the treatment of acute. The reduction of the alveolar volume is worsened during expiration. The lungs lose compliance, the FRC reduced and there is a substantial reduction of alveolar capillary membrane required for adequate gas exchange. Patients presenting with cardiogenic acute pulmonary edema (congestive heart failure) are experiencing several acute derangements of their pulmonary function caused by fluid being forced through the alveolar capillary membrane and into the interstitial spaces. The skin has become less diaphoretic and his work of breathing improved. The respiratory frequency dropped to 32/minute S02 – increased to 88% BP – 144/88 and the skin color has gone from cyanosis to pale with no cyanosis. A very modest reduction in work of breathing noted. Complications associated with their use relate to the pressures applied and include pulmonary barotrauma, decreased cardiac output and raised intracranial pressure. Begin CPAP via facemask at +5 cmH20 at an FIO2 of 40%. PEEP/CPAP may be applied using endotracheal tubes, nasal masks or prongs, or face masks or chambers to treat a wide range of adult and paediatric respiratory disorders.(restoration of functional residual capacity improve alveolar filling and improve pulmonary compliance.) To improve oxygenation and respiratory function. CPAP: Continuous Positive Airway Pressure PCV: Pressure control ventilation (similar to pressure control on an invasive ventilator with set rate) PSV. Pump failure induced acute pulmonary edema with severe masked hypoxemia and early respiratory failure.
